Most New Ebola Cases in Congo Result from Unknown Transmission Routes

BUNIA, DEMOCRATIC REPUBLIC OF THE CONGO / RankWire.AI / – The World Health Organization has reported that 80% of recent Ebola cases in eastern Congo are linked to unidentified transmission chains. Many patients did not appear on contact lists prior to testing confirming their infection. This gap hampers early isolation efforts and delays treatment for symptomatic individuals. Often, health teams only identify new clusters after patients seek care at clinics or pass away in their communities. The ongoing outbreak involves the less common Bundibugyo virus species.

Unknown transmission chains drive most new Congo Ebola cases
Congo’s Ebola outbreak has passed 2,000 confirmed cases across five provinces.

By July 13, Congo had documented 2,011 confirmed cases and 754 fatalities. The primary hotspot remained Ituri province, with 1,808 cases and 631 deaths. North Kivu reported 182 cases and 106 deaths, while South Kivu, Haut-Uele, and Tshopo also confirmed infections. Authorities listed 753 individuals in isolation and noted 366 recoveries. Response teams monitored roughly 67% of identified contacts across the most affected regions.

Contact tracing is crucial in identifying exposed individuals before the virus spreads further. Teams typically observe each contact for 21 days following their last known exposure. WHO stated that 92.3% of the 430 investigated deaths up to July 5 occurred outside hospitals or prior to admission, reducing early testing and isolation opportunities. Ebola transmits through direct contact with infected blood or bodily fluids, and contaminated objects can also serve as transmission sources.

    Five provinces report confirmed cases

The outbreak has affected 45 health zones across five provinces in Congo. Ituri has recorded cases in 26 zones, North Kivu in 11, Haut-Uele in 14 with 13 deaths, Tshopo with four cases and three deaths, and South Kivu with three cases and one death. The widespread geographic distribution has increased the burden on laboratories, treatment centers, and mobile surveillance units.

In Uganda, 20 cases and two deaths had been confirmed by July 14. Seventeen of those patients recovered, and the latest case was recorded on June 21. Of the infections, 15 were linked to travel from Congo, while five cases resulted from local transmission events. Ugandan health authorities reported no evidence of community-wide spread and continued monitoring of travelers and aid workers leaving affected regions during the outbreak.

Expansion of testing and clinical response

There is no licensed vaccine or approved treatment specifically targeting Bundibugyo virus. Medical teams focus on providing supportive care, including fluids, oxygen, and electrolyte replacement. WHO added the first molecular diagnostic test for this virus to its Emergency Use Listing on July 2. Currently, ten laboratories support testing in the affected area, capable of conducting over 2,000 tests daily. Additionally, a clinical trial involving remdesivir and the antibody treatment MBP134 has been initiated.

The Congolese government, WHO, and Africa CDC continue to coordinate efforts related to surveillance, testing, treatment, safe burials, and community outreach. Challenges such as insecurity, displacement, frequent movement in mining areas, and strikes by health workers have complicated response efforts. WHO has received approximately 40% of its $115 million funding appeal. Authorities emphasize the importance of rapid case detection, as most new infections are still occurring outside known transmission chains.
